Welcome to the online registration for the 2025/26 AWANA year at Bethel Baptist in Graham. 

Thank you for registering your child for the upcoming AWANA year. We look forward to another exciting year as your child grows in his/her knowledge of God's Word. Please fill out the following informaiton the best you can.

Puggles(2yrs old):  1 year program
                            $40 -Shirt, backpack, lesson cards
                             and yearly dues

Cubbies(3-4 yrs old):  2 year program
                                1st year - $45 -includes Vest, Book,
                                bag and yearly dues
                                2nd year - $26 Book and dues
                                 
Sparks(5-8 yrds old):   3 year program
                                 1st year $45- Vest, Book, bag and
                                 yearly dues 
                                 2/3year  $26 - Book and yearly dues

T&T:(9-12 years old)    3 year program
                                  1st year $55 - Book, Shirt, bag and dues
                                  2/3year $26 - Book and yearly dues

We have added the ability to pay for these groups online this year. 

Please Sign and Date the Registration Liability, Consent, and Release Form below. (Make sure to read the following statement carefully before subitting.)

By doing so, you, the undersigned parent/guardian of the above child, consent to him/her participating in AWANA Clubs , an event sponsored by Bethel Baptist Church (BBC) Sunday Evenings from Sept. 7th, 2025 through May 3rd, 2026. I ceritify that my child is able to participate in all activities. If my child has mediacl conditions which may be relevant to a physician in the event of an emergency, I have listed them in the Emergency Medical Information section of this registration form. If a medical emergency should arise regarding the afore mentioned child, I hereby give permission to the ministers, counselors, or representatives of BBC to select a physician and/or hospital for my child's care and to administer any emergency medical treatment that my child may require. I also give such medical personnel and/or hospital my permission to treat my child at the request of representatives of BBC. I understand BBC will not assume responsibility for medical expenses for my child and agree to bear such responisbility and pay any such expenses incurred with respect to such medical emergency. I also relieve BBC, it's ministers, counselours, and representatives from any claim/or liability or damages as a result of any illness or injury, including death, or any property damagewhich may be suffered by my child during scheduled activities of BBC, including, but not limited to, the events listed above. Also, including transportation in conncection therewith., regardless of the cause thereof. I further acknowledge that the representatives of BBC shall not be resposnilbe for supervising the takig of any medication required by my child during such trips or activities. I UNDERSTAND THAT I CAN REVOKE THIS RELEASE AT ANY TIME WITH A WRITTEN REQUEST.

I, the below signed parent/gaurdian, release that Bethel Baptist Church in not responsible for personal belongings of the above listed child.

I, the below signed parent/gaurdian, being the legal parent/gaurdian of the above listed child acknowledge that the information listed on this form is given to the best of my knowledge and I hereby give my permission for mim/her to go, travel, and to particiapte in any activity as mentioned above under the direction of Bethel Baptist Church AWANA Ministries.

PHOTO & VIDEO RELEASE - PLEASE SIGN AND DATE IF YOU AGREE

I give permission for Bethel Baptist Church to use photos and videos involving my son/daughter in the use of appropriate publication material on social media and the church website.
